Job summary

Southern Kentucky Rehabilitation Hospital, a joint venture with Select Medical, is seeking a Case Manager to coordinate patient care and discharge planning at our Bowling Green, KY location on a Per Diem schedule. You will collaborate with clinical teams to ensure quality outcomes and cost-effective care.

The ideal candidate holds state licensure in a clinical discipline or a Bachelor's/Master's in health or human services, with preferred CCM certification and prior discharge planning

Qualifications

• Current Licensure per state guidelines in clinical or related discipline OR a Bachelor's or Master's in a health or human services discipline.
• Previous experience in Case Management and Discharge Planning preferred.
• CCM Certification Preferred.

Responsibilities

• Coordinate health care decisions to improve quality and outcomes.
• Coordinate care across the continuum; promotion of cost-effective care within the allotted time frame.
• Assure payments of hospital-based services meeting patient-related utilization management criteria; implement discharge plans.
• Assess psychosocial needs of the patient and provide discharge planning interventions.

Responsibilities

The Case Manager is responsible for the coordination of health care decisions by using a systematic approach to assure treatment plans that improve quality and outcomes, coordination of care across the continuum; promotion of cost-effective care within the allotted time frame; assuring payments of hospital-based services meeting patient-related utilization management criteria, and implementation of safe and appropriate discharge plans. The Case Manager assesses the psychosocial needs of the patient and provides intervention as part of the discharge planning process.

Qualifications

Minimum Qualifications
