Job summary

Neulife Rehabilitation of Florida is seeking a skilled Case Manager at our Mt. Dora location to coordinate resident care, manage insurance authorizations, and facilitate discharge planning within an interdisciplinary team.

You will advocate for residents and families, monitor progress, ensure timely documentation, and collaborate with physicians, therapists, and insurers to remove barriers to care in a fast-paced rehabilitation setting.
