Job summary

Blue Sky, Inc. is seeking a Part-Time Case Manager to join our team in Detroit Lakes, MN. This role offers 20-25 hours per week, Monday through Friday between 8:00 a.m. and 4:30 p.m., with responsibilities to help individuals access services, develop person-centered plans, and build community connections.

You will coordinate supports, monitor services, and educate clients and their legal representatives about options while promoting independence and community integration.

Qualifications

• Bachelor's degree in Social Work, Psychology, Sociology, or related Human Services field.
• Minimum 1 year of experience working with individuals with developmental disabilities or related conditions.
• Strong self-motivation, organization, and problem-solving skills.

We are seeking a Part-Time Case Manager to join our team. This position offers 20-25 hours per week (Mon - Fri between 8a - 4:30p) and provides an opportunity to support vulnerable individuals and their families by helping them access services, achieve personal goals, and build meaningful connections within their communities.

As a Case Manager, you will:

• Coordinate services and supports for individuals and families.

Qualifications

• Successful applicants will be able to manage priorities effectively, work independently, and collaborate as part of a team.
• Bachelor's degree from an accredited four-year college or university in Social Work, Psychology, Sociology, or another Human Services-related field.
• One year of experience in working with individuals with developmental disabilities or related conditions
