Program: Domestic Violence Center
Status: Part-Time, Hourly position.
Description:
Position is responsible for providing quality client care by linking clients with community resources, advocating in meeting client needs and completing documentation. The Case Management activities will vary by residential program and client need. This position follows the site S.O.P. in all day to day operations.

Sunday Off
Monday 8:00am-12:00pm
Tuesday 800am-12:00pm
Wednesday 8:00am-12:00pm
Thursday 8:00am-12:00pm
Friday 8:00am-12:00pm
Saturday Off
ESSENTIAL DUTIES AND RESPONSIBILITIES Include The Following.
Acts as a liaison between clients and external agencies and other internal Synergy resources. (i.e. medical services, school, external social workers, etc.) Assists clients with telephone calls to resources as requested and/or needed. Completes correspondence as required.
Handles the coordination of agency efforts to support individuals in need of housing resources across the community.
Assists clients, as needed for program, in locating assistance for housing, utilities, and medical services, employment search and seeking affordable daycare. Assists clients with clothing needs for themselves and family members as needed.
As program requires, meets weekly with clients to set goals and monitor client’s progress.
Transports clients to necessary destinations.
Assists clients in activities related to their housing.
Facilitates educational groups for clients as required by program.
Assists clients in crisis.
Oversees client behaviors and activities. Intervenes and provides assistance as needed.
Dispenses medications to clients as required according to med tech certification guidelines.
Completes client intake and discharge paperwork as needed. Completes discharge summaries as required. Maintains accurate documentation of client record and maintains files in accordance with Synergy standards. Completes documentation both accurately and professionally in providing necessary information about a client’s stay. Communicates via com log, behavioral logs, etc. to ensure advocates have necessary information to meet clients needs.
Documents daily on client activities in client files.
Maintains communication with other staff members – both oral and written.
Attends community/agency case manager meetings as required and relays relevant information to staff.
Attends staff meetings and weekly case review.
Performs general clerical duties, answers telephone and hotline calls as required by program.
Attends internal and external trainings and in-services to meet the required 40 hour per year licensing requirement, as long as within department budget and staff/client ratios are met.
Maintains client confidentiality.
Acts as the on-call supervisor and makes appropriate decisions as required/needed when serving in the on-call rotation as required.
Participation in Performance and Quality Improvement activity is required by each employee.
Performs other duties as assigned.

Qualifications

EDUCATION And/or EXPERIENCE
Bachelor's degree in Social Work, Psychology, Counseling or related field from an accredited college or university. Position requires one to two years prior case management experience. Prior experience working with survivors of domestic violence preferred. Having knowledge of community resources a plus.
Language Skills
Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public.
Mathematical Skills
Ability to calculate figures and amounts such as discounts, interest, commissions, proportions, percentages, area, circumference, and volume. Ability to apply concepts of basic algebra and geometry.
REASONING ABILITY
Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
CERTIFICATES, LICENSES, REGISTRATIONS
The employee in this position must have a valid Class E Drivers license. The employee in this position must have CPR/First Aid Certification and be a Medical Technician.
Other Skills And Abilities
The employee in this position must have the ability to transport up to fifteen (15) passengers in a van. The employee in this position must have the ability to type and perform Data Entry/10 key, and use computer software, including Windows, Word for Windows, Alice, and Access.

Must be at least 21 years of age, pass a background check and Family Care Safety Registry screening, fingerprint screen, drug screen and have a valid driver’s license and proof of insurance. Position requires CPR, Med Tech Certification and Chauffeur’s License within 30 days from the date of hire.
