LifeMoves is the largest and most effective provider of housing and services for neighbors experiencing homelessness in Silicon Valley since 1987. Our Mission is to end homelessness by providing interim housing, supportive services, and building collaborative partnerships. With 40 programs, LifeMoves gives our neighbors experiencing homelessness a temporary place to call home while providing intensive, customized case management through site-based programs and community outreach.
SHIFT – Monday-Friday 8am-4:30pm. LifeMoves partners with emergency departments at four Kaiser Permanente hospitals across Santa Clara and San Mateo Counties. Each of the four hospitals has a LifeMoves case manager assigned to support their emergency department. This position will be assigned to Kaiser Permanente Santa Clara. The team connects individuals experiencing homelessness to community resources, housing support, and medical care at the time of hospital discharge. The program’s goal is to reduce hospital readmissions, improve health outcomes, and increase housing stability through compassionate and coordinated support.
