Disability Services of the Southwest is seeking a dedicated Case Manager to join their Beaumont team. This role involves overseeing operations of their managed waiver program, supporting individuals with disabilities, and building strong relationships with clients and families.

Qualifications

  • Bachelor’s in health/human services + 2 years’ experience.
  • Associate’s + 4 years’ experience.
  • High school diploma + 6 years’ experience.
  • Reliable transportation with a valid TX driver’s license.
  • CPR/First Aid certification.

Responsibilities

  • Oversee day‑to‑day operations of the managed waiver program.
  • Monitor and support services for your assigned caseload.
  • Enroll new applicants and ensure compliance with DBMD program standards.
  • Build strong relationships with clients and team members.
  • Provide crisis intervention and on‑call support as needed.

What You’ll Do:
  • Oversee day‑to‑day operations of the managed waiver program.
  • Monitor and support services for your assigned caseload.
  • Enroll new applicants and ensure compliance with DBMD program standards.
  • Build strong, positive relationships with clients, families, vendors, and team members.
  • Provide crisis intervention and on‑call support as needed.
  • Travel to client homes and community activities in the Beaumont and Houston areas.
What You Bring:
  • Education & Experience (one of the following):
    • Bachelor’s in health/human services + 2 years’ experience
    • Associate’s + 4 years’ experience
    • High school diploma + 6 years’ experience
  • Strong communication skills and fluency in client communication methods.
  • Proficiency in MS Word, Excel, and database systems.
  • Reliable transportation, valid TX driver’s license, and auto insurance.
  • CPR/First Aid certification (in‑person only).
