Job summary

Guest House in Milwaukee is seeking a Case Manager for the My Home Housing Program. The role coordinates services, performs client assessments, and maintains documentation while being available for on-call emergencies.

Responsibilities include home visits, budgeting support, resource referrals, and collaboration with landlords and collateral contacts. A bachelor’s degree in social work or related field is required, with strong communication and driving eligibility.

• Maintain a valid Wisconsin driver's license, vehicle insurance, and reliable transportation in compliance with agency requirements.

• Obtaining and maintaining necessary client paperwork.
• Performing referral and exit paperwork for clients.
• Checking both voicemail and e-mail regularly.
• Coordinating and delivering needed donations to clients.
• Tracking mileage and turning in mileage log to Lead Case Manager each month.
• Maintaining relationships with landlords and responding to clients' needs by communicating with landlords.
• Utilizing training opportunities offered by the Guest House.
• Participating in an annual evaluation with the Lead Case Manager.
• Other duties as assigned by the Director of Community Housing.

Job Title: Case Manager - My Home (SPC)

Reports To: Director of Community Housing Programs

FLSA Status: Full-time/Salaried

This position is responsible for the coordination, planning and implementation of services for My Home Housing Program participants. The Case Manager is expected to be available on a 24-hour "on-call" basis for emergencies.

• Bachelor's Degree in Social Work, Human Services or a related field required.
  • In lieu of a Bachelor's degree, an Associates degree, and 1 year of experience working with homeless population is acceptable.
