The role emphasizes empathy, thorough documentation, and coordination with physicians and nursing staff to ensure safe, person-centered discharge planning and ongoing support.

Qualifications

• Master’s degree in social work (MSW), Psychology, Counseling, or a closely related health field required.
• Minimum of two years of professional case management or social work experience.
• Compassionate with excellent communication and problem-solving skills.
• Proficient with electronic health records (EHR) and basic office software.
• Experience with therapeutic limit-setting or understanding of level systems or behavior management modalities helpful.
• Ability to maintain professional boundaries.

Responsibilities

• Conduct detailed interviews to identify psychological, medical, and social needs.
• Develop specific, step-by-step care plans to help clients reach goals.
• Provide individual, group, and family psychoeducation-oriented care using various treatment modalities.
• Coordinate discharge planning for patients with physician.
• Communicate legal requirements for case management with physician and nursing staff.
• Document treatment provided in medical records.
• Report incidents of suspected abuse using prescribed forms and protocols.
• Link patients to housing, food assistance, healthcare, and financial aid programs.
• Document all client interactions and maintain confidential files.
