Acenda Integrated Health is seeking a Case Manager to provide case management, outreach, and supportive housing assistance for youth in Burlington County through the MRSS program. The role focuses on safety, stability, mental health, and access to services, with crisis support and counseling as part of daily duties.

Responsibilities include coordinating with vocational/educational partners, assisting with crisis management, and helping youth secure public benefits and recovery strategies.

Qualifications

• Bachelor’s degree in social service-related field required.
• 1 year of experience with children, youth or families.
• Knowledge and/or experience in trauma-informed care, mental health and substance abuse.
• Must have and maintain a valid driver’s license, use of an insured vehicle and an acceptable driving record.

Job Overview

The Case Manager provides case management, outreach, and individualized support to youth in or seeking supportive housing, ensuring their safety, stability, and access to services addressing mental health, substance use, domestic violence, economic challenges, and other needs. The role also includes coordinating with vocational and educational partners, offering counseling and crisis support, assisting with medication monitoring, and helping youth secure public benefits and develop recovery strategies.

Program Info

Children’s Mobile Response and Stabilization Services (MRSS) is an in-community crisis response to young people ages 0-21 who are in crisis as that crisis is defined by the family. Mobile Response Workers provide immediate crisis de-escalation and stabilization services. Support is available 24 hours a day, 7 days a week and we can come into your home, or provide support by phone or video. Crisis interventions include de-escalation, service linkage, and short term stabilization management.

Key Responsibilities:

• Provide case management, outreach, engagement, and support to youth residing in or in search of supportive housing units in Burlington County.
• Ensures the safety, stability and well-being of youth and effectively work with and identify services/supports for youth who may be facing a range of co-occurring challenges, including mental health, domestic violence, substance use, economic challenges, lack of support, and other needs
• Liaison with vocational and educational community partners to help youth develop career plans and obstacles to employment.
• Provide individual support and counseling and assist with crisis management and medication monitoring
• Help youth secure public benefits, and develop recovery strategies.

Requirements:

• Bachelor’s degree in social service- related field required.
• 1 year of experience in the field working with children, youth or families.
• Knowledge and/or experience in trauma-informed care, mental health and substance abuse.
• Must have and maintain a valid driver’s license, use of an insured vehicle and an acceptable driving record.

Additional Information:

• Base Rate: $19.23
• Sign-on Bonus: $1,000
• Bilingual: An additional $1.50 added to the base rate if Spanish proficient
• Schedule: Two shifts 8:30-4:30p; one shift 11-7p; two shifts 1-9p plus on-call overnights, weekends, and holidays in a rotation
