Job summary

Qualifications

Responsibilities

• Manage treatment activities for patients, including facilitating therapy groups on patient units.
• Complete thorough assessments and develop treatment plans within a multidisciplinary team.
• Coordinate care with external providers and managed care organizations.
• Plan and implement discharge planning with risk assessment and follow-up resources.
• Communicate and exchange information with the treatment team and collateral contacts.
• Demonstrate organization, flexibility, and responsiveness to feedback.
