Job summary

The Sierra Vista Hospital location, a 171-bed psychiatric care facility located in south Sacramento, is a full-service mental health facility and a center of support for patients and their families.

The Case Manager is responsible for managing the treatment activities offered to the patients, which may include facilitating therapy groups on patient units; complete thorough assessments; assessing patients' needs and compiling a treatment plan within a multidisciplinary team; planning and/or

Qualifications

• Master's Degree in social work, counseling psychology, mental health or related field, with transcripts.
• One year direct clinical experience in a psychiatric/mental health setting preferred.
• Complete CPI and CPR certification within first 90 days of employment.
• Experience in assessment, risk and safety assessments, treatment planning, and coordination with external review orgs.

Responsibilities

• Manage treatment activities for patients, including facilitating therapy groups on units.
• Complete thorough assessments and develop treatment plans within a multidisciplinary team.
• Plan and provide care, treatment and services; coordinate with external providers and managed care.
• Implement discharge planning with risk assessment and arrange follow-up resources.
• Communicate with treatment teams and adapt to department needs; accept feedback and improve daily practices.
