Job description

Under the general supervision of the Program Director or Designee, functions as part of a team providing client-centered, strength-based, trauma-informed rehabilitation and recovery services to consumers diagnosed with a mental illness, and in some cases, co-occurring substance abuse disorder. These services focus on supporting, instructing, and assisting recipients of mental health services to develop skills needed to attain personal goals, and to live, work, and socialize successfully in their chosen community environments. This includes hands-on assistance with daily living skills such as hygiene, upkeep of physical space and living environment (including bed-bug prevention and treatment), laundry, shopping, meal planning and preparation, cooking, housekeeping, and traveling.

ESSENTIAL TASKS:

To perform this job successfully, an individual must be able to perform each essential duty listed satisfactorily. Reasonable accommodations may be made to enable qualified individuals with disabilities to perform the essential functions unless this causes undue hardship to the agency.

  1. Secure information related to medical, psychological, and social factors contributing to the individual's situation, evaluate these factors, and complete and maintain consumer treatment plans, including quarterly service-plan reviews, assessments, medical records, and changes in treatment and/or medications, either manually or electronically as instructed.
  2. Provide supportive counseling in 1:1 or group formats to assist consumers in modifying attitudes and behaviors as needed.
  3. Maintain up-to-date, accurate individual case records, develop measurable and objective service plans that maximize consumer rehabilitative abilities, and document progress. Review service plans regularly and perform follow-up to ensure quality of services. Ensure plans are culturally and linguistically appropriate.
  4. Facilitate individualized services that meet diverse consumer needs and focus on discharge planning. Document significant events and contacts with mental health providers.
  5. Model appropriate behavior and deliver rehabilitation and recovery services incorporating skills training in psychiatric rehabilitation, including the 11 restorative services, using cognitive-behavioral techniques such as role-plays and plans of action. Skills training areas include self-advocacy, ADL, medication management, socialization, and conflict resolution.
  6. Ensure consumer compliance with medication and medical appointments, including routines, to meet treatment goals.
  7. Monitor medication self-administration, count doses, and maintain medication records following agency policies.
  8. Accompany consumers to medical, social, or government appointments as needed.
  9. Gather and disseminate resource information for consumers, advocating on their behalf and coordinating activities such as social, recreational, vocational, and medical appointments.
  10. Perform crisis assessments and interventions using approved techniques.
  11. Participate in safety drills, fire drills, and other safety activities with team members.
  12. Report serious incidents immediately, complete incident reports, and manage missing person reports as per policy.
  13. Conduct room checks and inspections to ensure safety, accountability, and cleanliness. Engage consumers in maintaining a safe and healthy living environment.
  14. Participate in program communication and training activities, including shift briefings, meetings, and community events.
  15. Execute emergency plans according to policy.
  16. Assist in the orientation of new personnel and support supervisors and managers with reports and projects.
  17. Function as floor supervisor as assigned, overseeing staff, ensuring compliance with safety protocols, and responding to emergencies.
  18. May cover duties of other staff in cases of short staffing.
  19. Maintain reliable attendance and comply with agency policies on infection control and on-call responsibilities.
  20. Perform other duties as assigned.

MINIMUM EDUCATION AND EXPERIENCE REQUIREMENTS:

Bachelor's degree in a human services field, or

Associate’s Degree in a human services field and two years of relevant experience, or

High school diploma and four years of relevant human services work, or an equivalent combination of training and experience. Up to 30 college credits can substitute for one year of experience.

Degrees must be from accredited institutions or recognized by the NYS Education Department. Foreign degrees require verification of equivalency.

CASE MANAGER TRAINEE: Candidates lacking experience may be hired as trainees at a reduced salary until requirements are met.

DESIRABLE KNOWLEDGE, SKILLS, AND ABILITIES:

  • Basic knowledge of mental illness, emotional disturbances, and substance abuse disorders.
  • Basic understanding of treatment, rehabilitation, and community support programs.
  • Knowledge of clinical procedures and medications.
  • Basic crisis management techniques.
