BRISTOL PLACE CORPORATION is seeking a FT Case Manager in Minnesota to assess and coordinate care for individuals with CADI, DD, and BI waivers. You will develop and review care plans, and connect clients with appropriate community resources to meet deadlines and guidelines.

Qualifications include a 4-year degree in a Human Services field and at least 1 year of relevant experience. A valid driver’s license and reliable vehicle are required for travel within the Twin Cities/Metro area.

Qualifications

• 4 year degree in Human Services Field (Social Work, Psychology, Sociology) or closely related field.
• 1 year experience working with individuals diagnosed with Mental Illness and/or Developmental Disability.
• Driver’s license, reliable vehicle and proof of insurance.
• Reliably commute or relocate prior to hire in order to work with individuals in the Twin Cities/Metro Area.
• Must clear background check.
• Candidate should have good verbal and written communication, be organized, flexible and motivated.
• Strong time management and multitasking skills are essential to this position.
• Experience working with CADI, BI and/or DD waivers preferred.

HOURS AVAILABLE: Monday - Friday 8:00am - 4:30pm, with some flexibility. Starting wage $45,000+ based on education and experience.

WORK LOCATION: Primary location is North St. Paul, MN. Travel throughout the Twin Cities/Metro Area is required for meetings and face to face visits with individuals in the community. Based on the caseload, there may be travel outside the Metro Area.
