Case Manager– LSW, LSA, or RN Certified

Job Summary:

We are looking for a compassionate and organized Case Manager certified as an LSW, LSA, or RN Case Manager to support individualized care for patients with developmental disabilities. The role includes care coordination, community resource navigation, and multi-agency collaboration to support patient success after discharge.

Key Responsibilities:

  • Support discharge planning and coordinate services across care teams.
  • Assist patients and families in accessing housing, education, and medical services.
  • Participate in case reviews, outcome tracking, and interdisciplinary meetings.
  • Educate patients and families on life skills, insurance, and adult services.

Required Experience:

  • 3+ years of full-time or equivalent part-time case management experience.
  • Active LSW, LSA, or RN Case Manager certification.
  • Proficient in working with families and social service systems.
  • Familiarity with state and local agency operations.
  • Ability to work in Canton for a Monday-Friday day shift.
