Case Manager, Law Enforcement Assisted Diversion (LEAD)

Position Summary

The Law Enforcement Assisted Diversion (LEAD) initiative for the City of Pittsburgh aims to provide individuals with substance use disorder and mental/behavioral health challenges with person-centered social services and intensive case management focused on harm reduction and compassion, instead of criminalization. The goal is to reduce legal recidivism and improve quality of life through supportive services. The position involves engaging with individuals experiencing homelessness, poverty, mental/behavioral challenges, active drug use, and suspected low-level offenses, including non-violent crimes and prostitution. The Case Manager will conduct outreach, provide long-term engagement, and collaborate with LEAD partners, service providers, housing providers, and community organizations.

This position is grant-funded for one year, with future funding contingent on grants and other sources.

Department: Public Safety, Office of Community Health & Safety

Salary: $62,670 per year

Requirements

Applicants must submit a complete application including education, work experience, a resume, and supplemental questions. Residency in Pittsburgh is required throughout employment. A valid Class C Pennsylvania Driver's License is also required. Candidates on the current eligibility list do not need to re-apply. Background checks and verification of education are mandatory prior to employment.

Qualifying Requirements
  • At least three years of full-time experience in social services, criminal justice, healthcare street outreach, or related fields.
  • High school diploma or equivalent; a bachelor’s degree is preferred.
Position Duties
  • Provide outreach and intensive case management services.
  • Engage with participants on the street and at social service facilities.
  • Assess participants' needs, including substance dependency and housing status.
  • Assist participants in accessing services like housing, funding, and healthcare.
  • Develop and update Individualized Service Plans (ISP).
  • Maintain participant files and advocate within the criminal justice system.
  • Collaborate with community providers and LEAD partners.
  • Identify systemic gaps and barriers, and participate in meetings.
