Quality Moments Behavioral Health is seeking a Senior Care Case Manager to provide therapeutic activities and companionship for seniors in skilled nursing facilities. This role can be filled as part‑time or full‑time, offering compassionate, reliable support with a focus on dignity and personal connection.

The ideal candidate will have a heart for elder care, be dependable, and capable of building meaningful relationships while delivering social, emotional, and behavioral well‑being activities

Qualifications

• Must be at least 21 years of age.
• Desire to improve quality of life for aging adults.
• Prior experience in social work, counseling, or related field preferred.
• Experience with Alzheimer patients preferred.
• Valid driver’s license and transportation required.
• Previous EHR/documentation experience helpful.

Responsibilities

• Provide therapeutic activities and companionship to individuals in skilled nursing facilities.
• Engage seniors in social, emotional, and behavioral well‑being activities.
• Build meaningful relationships and deliver person‑centered support.

Qualifications and Responsibilities of Senior Care Case Manager

• Must be at least 21 years of age
• Diploma or GED required
• Desire to improve quality of life for aging adults.
• Must have experience working with Alzheimer patients.
• High level of commitment, dependability, and interpersonal skills.
• Prior experience as an STNA, CNA, or in a nursing or long-term care setting is preferred.
• Prior experience in social work, counseling, human services, or a related field is preferred.
• Valid driver’s license and reliable transportation with current automobile insurance meeting state minimum liability requirements.
• Previous EHR system use, and professional documentation experience are helpful.
