Responsibilities

  • Maintain ethical, trauma-informed communications at all times.
  • Travel within the top 21 counties to provide services for IDD individuals.
  • Assist clients transitioning from out-of-home placements back into the community.
  • Enroll individuals in Home and Community-based Services program.
  • Provide ongoing assessments, treatment planning, and documentation.
  • Participate in team meetings to develop annual PDP.
  • Link community resources to support independent living.
  • Complete documentation according to deadlines and policy.
  • Collaborate with other departments to meet client needs.
  • Help improve lives of individuals with IDD in home and community.
  • Adhere to the Code of Conduct and report concerns as needed.

Texas Panhandle Centers is seeking an Intellectual & Developmental Disabilities Case Manager (1117) to deliver trauma-informed care and skills training to adults (18+) across Amarillo and the surrounding service area of the Texas Panhandle. This role is ideal for human services graduates looking to gain direct clinical experience in a supportive, multi-disciplinary team environment.

Key Responsibilities:

  • Maintain ethical and professional communications at all times using respectful trauma-informed care language.
  • Travel within the top 21 counties of the Texas Panhandle to provide services for individuals with intellectual and developmental disabilities (IDD).
  • Assist children and adults with IDD as they transition from out-of-home placements, such as State Supported Living Centers or Nursing Facilities, back into the community.
  • Enroll individuals in the Home and Community-based Services program to support their transition back into the community from a facility-type setting.
  • Provide ongoing assessments, treatment planning, documentation related to the moves within the transition, and other case management activities.
  • Participate in team meetings to develop Person Directed Plan (PDP) annually.
  • Link resources in the community to aid individuals with living in the community as independently as possible.
  • Complete documentation according to program deadlines and company policies to comply with state regulations.
  • Work closely with other departments in the agency to ensure that each individual’s needs are met.
  • Make lives better for individuals with IDD in both their homes and in the community.
  • Adhere to TPC Business Code of Conduct and report concerns if ethical and/or business standards defined in the code are compromised.

Qualifications & Credentials:

  • Bachelor’s degree from an accredited university in Social, Behavioral, or Human Services field including, but not limited to, Psychology, Social Work, Medicine, Nursing, Rehabilitation, Counseling, Sociology, Human Development, Gerontology, Educational Psychology, Education, and Criminal Justice (required for QIDP credentialing).
  • Excellent time management skills and the ability to manage on own caseload independently.
  • Excellent computer skills including Microsoft Word and Outlook.
  • Excellent writing and verbal skills.
  • Organize documents for assigned caseload and meet deadlines.
  • Valid Texas Driver’s License with an insurable driving record.
  • At least 18 years of age.

Experience Level Tiering:

  • Case Manager II ($43,305.60/yr): Minimum 1 year of case management experience or working with the IDD population.
  • Case Manager III ($46,009.60/yr): Minimum 2 years of case management experience or working with the IDD population.
  • Graduate-level coursework in a related field may substitute for up to 1 year of required experience.

Preferred Qualifications:

  • Bilingual (English/Spanish) is a strong plus.
  • Knowledge of community resources.
  • Ability to assess client needs, coordinate client services, and monitor program effectiveness.
  • Experience with SmartCare EHR.
