The COD Counselor provides outreach, engagement, and ongoing case management for adults with mental health and substance use disorders. Responsibilities include crisis assessment, biopsychosocial evaluations, collaboration

Qualifications

• Bachelor’s degree in social work, psychology, or related field.
• Substance Use Disorder credential or ability to obtain within 60 days.
• One year of direct services with vulnerable populations.
• Excellent oral and written communication skills.
• Ability to maintain accurate records and work independently.

Responsibilities

• Outreach and engage individuals experiencing homelessness across CCS shelter and housing sites.
• Provide crisis de-escalation and risk assessment; advocate with responders when higher-level services are needed.
• Perform trauma-informed biopsychosocial assessments and assist with MH/COD assessments.
• Facilitate case management, therapy, and skills training to support recovery.
• Develop bi-annual treatment plans and document progress in case notes.
• Participate in psychiatric consultations, staffing, and supervision.
• Lead support groups on topics like housing, daily living skills, and recovery.
