Job summary

Dignity Health - St. Joseph’s Behavioral Health Center in Stockton, California seeks a Case Management Professional to guide patients through complex health journeys and coordinate resources for optimal outcomes.

You will assess needs, develop individualized care plans, facilitate transitions, and collaborate with multidisciplinary teams to ensure continuity of care and prevent readmissions. The role requires empathy, strong communication, and a deep understanding of healthcare systems.

Qualifications

• Masters Degree and Registered intern with BBS or eligible.
• Basic Life Support is required.

• Maintains privacy and modesty in the provision of care.
• Identifies self by name and title to patient/family.
• Determines presence/type of Advanced Directive.
• Reports suspected cases of abuse/neglect, if identified.

Job Summary and Responsibilities

As a Case Management Professional, you will be a pivotal coordinator and advocate, guiding patients through complex healthcare journeys to optimize outcomes and well-being.

Every day, you will assess patient needs, develop individualized care plans, facilitate seamless transitions, and coordinate resources. You'll educate patients and collaborate with multidisciplinary teams to ensure continuity of care and prevent readmissions.

To be successful in this role, you will possess exceptional assessment and coordination skills, strong communication, and a deep understanding of healthcare systems, with an empathetic approach to patient advocacy.

$5,000 Sign-On Bonus offered for new hires. Per policy, current employees are not eligible for the Sign-On Bonus. However, we are offering a Referral Bonus for this role.

Pay Range

$45.03 - $51.44 /hour
