Qualifications

  • Support TAY to navigate safety, health, and economic stability by teaching self-advocacy and using effective strategies.
  • Identify knowledge gaps and link TAY to educational resources to increase essential skills.
  • Coordinate, facilitate, and/or conduct expressive arts activities or psychoeducation groups.
  • Monitor client progress and maintain records in required databases and files.
  • Participate in staff meetings and community meetings to strengthen partnerships.

Position Summary

The Transitional Aged Youth Health & Wellness Case Manager outreaches to, intakes, assesses, counsels, and coordinates care for TAY ages 18-27 years old to achieve safety, health, and well-being across multiple life domains such as housing/shelter, education and employment, behavioral health, physical health, community building, family and relationships, and much more. The Case Manager serves as the client’s main advocate and care coordinator within GLIDE’s continuum of care, community partners and resources, and other external systems of care. This position works closely and collaboratively with other TAY HWC team members to enable warm hand-offs and linkages amongst specialists, case managers, navigators, and ambassadors. Upon exit from GLIDE’s care, the Case Manager develops a care transition plan to ensure the clients are linked to additional support services and knows how to reconnect with GLIDE for future support. In addition to 1:1 case management support, this position will also facilitate and/or coordinate weekly activities or psychoeducation groups that will support, promote, and enhance clients’ confidence, wellness, community connection, and growth mindset.

Work schedules may include the need to work on one day, during the weekend. Working hours may also vary, and occur during non-traditional hours.

  • Administer intake and assessments to enroll TAY into GLIDE’s continuum of care, establish baseline of safety and health needs, and develop individual goal/service plans that include actionable steps for care coordination.
  • Develop a care coordination plan based on 1:1 sessions with client, referral information, and/or assessment and i identify key care coordination needs within GLIDE’s integrated services (e.g. Health Access, Family Resource Center, Meals, etc.) including GLIDE partners (e.g. UCSF, Street Medicine, Clean Slate, Larkin St.) and establish successful linkages to care.
  • Support TAY to navigate and overcome barriers related safety, health, and economic stability by teaching and coaching TAY on how to self-advocate complex service systems using effective communication tools and strategies.
  • Identify personal knowledge and skills gaps that may negatively affect TAY ongoing stability and success and link TAY to educational resources to increase essential knowledge and skills, including but not limited to sexual health education, personal hygiene, pet care, harm reduction principles, building healthy relationships, adult literacy, digital literacy, interfacing with banking institutions, etc.
  • Coordinate, facilitate, and/or directly conduct expressive arts activities, therapeutic activities or psychoeducation groups that will support, promote, and enhance clients’ confidence, wellness, community connection, and growth mindset.
  • Monitor client progress through regular scheduled appointments and follow-up to ensure successful implementation of individual service plan and maintain accurate, timely, and up-to-date records of TAY intake, assessments, progress, and outcomes in required databases and hard copy files as required.
  • Participate in regular staff meetings, staff training events, supervisory meetings, and community meetings to build and maintain strong relationships with other GLIDE programs, community partners, and City agencies to enhance service delivery and client support.
  • Bachelor’s degree in social work, public health, community health, or related field preferred.
  • 5+ years of experience in providing intensive case management to TAY and/or similarly disenfranchised and high need populations in social service settings.
  • Experience and demonstrated success providing 1:1 counseling, case management.
  • Knowledge of Bay Area San Francisco TAY needs and resources, especially those experiencing poverty, homelessness, mental health challenges, substance misuse, trauma, and other associated experiences and how to navigate and link TAY to those resources successfully.
  • Must have e xperience facilitating the following activities and workshops such as weekly activities or psychoeducation groups that support and enhance clients’ confidence, wellness and community connection.
  • Ability to work independently, demonstrate initiative, be dependable, and have consistent presence.
  • Ability to collaborate with internal and external stakeholders to coordinate care for shared clients to help clients stabilize and reach their individual goals.
  • Strong written and oral communications skills, including excellent interpersonal skills.
  • Attention to detail, strong organizational, time management, and problem-solving skills.
  • Ability to use computer and reporting systems including Windows operating system, Microsoft Office Suite, Salesforce or other database systems, Zoom (or other virtual meeting systems), and search engines for research.
Work Environment

GLIDE’s buildings are located in the Tenderloin neighborhood.GLIDE maintains an "open door" policy to the community and its clients, who frequently enter and leave the building.
