Our Outpatient services specialize in treatment mental health and substance use disorders with treatment and skills groups, as well as community resources.
Outpatient programs provide trauma-informed care aimed at reducing the harmful impacts of substance use. Participants and their counselors work collaboratively to design an integrated, individualized, strengths-based treatment experience geared towards addressing each client’s specific needs and goals. Outpatient programs provide Individual and group therapy, individualized case management, treatment planning, drug and alcohol counseling, crisis intervention, and connection to social support services.

The Outpatient Program is specifically designed for people who struggle with substance use disorders, mental health issues or co-occurring disorders. The Outpatient Program works with high-risk populations from a variety of backgrounds. Our programs are open and welcoming to adult participants, of any gender identity. We accept the majority of participants who seek Outpatient treatment, including walk-in’s, referrals from other agencies and step-down transitional clients from residential treatment programs.
The Case Manager II is responsible for assessing treatment needs; monitoring and evaluating problem lists and progress and facilitating interdisciplinary approaches. The Case Manager II coordinates an approach to the delivery of health, substance use, mental health, and social services, linking clients with appropriate services to address specific needs and achieve stated goals. The Case Manager II’s duties include care coordination, maintaining positive working relationships with outside community partners, scheduling appointments, arranging transportation, assisting with acquiring medications, ongoing care and discharge planning, and crisis intervention.
Key Responsibilities
- Carries a caseload and meets service delivery goals.
- Work as an active team member and closely collaborate with team members.
- Help clients get and stay engaged in SUD treatment.
- Help clients transition between levels of care.
- Assess for opioid overdose and ensure beneficiary has Naloxone if appropriate.
- Interface with a multidisciplinary team in treatment planning and service delivery.
- Develop and assess the effectiveness of individualized treatment and consumer progress.
- Maintain documentation in compliance with agency, HIPAA, and DPH standards.
- Assist in the ongoing maintenance of consumers’ charts and other related documentation.
- Perform other duties as assigned by the Program Manager.
- Arrange work schedule in accordance with the agency’s needs.
- Comply with the agency’s policies and procedures.
- Attend internal and external meetings as assigned.
- Create a safe space for clients and maintain appropriate boundaries.
- Meet expected performance standards as assigned by supervisor.
- Provide “whatever it takes” services to consumers including providing services where the consumer lives, assisting with housing, benefits, employment, education, transportation, childcare, medical and other needed services.
- Provide case management services to clients as assigned.
- Serve as a care coordinator for clients as assigned. Responsibilities include coordination of all services with other providers and completion of all coordinated care documentation.
- Provide case management services to clients as assigned.
- Assist client in developing independent living skills to promote independence and self-sufficiency.
- Assist in crisis and symptom management.
- Acts as buddy to new hires.
Qualifications
Education, Certification, and Experience
- High school diploma or GED.
- AOD Registration + 2 years of case management experience.
OR
- AOD Certification
- Valid CA Driver’s License.
- First Aid and CPR certification.
- Experience working with the criminal justice population preferred.
- Experience working successfully with issues of substance abuse, mental health, criminal background, and other potential barriers to economic self-sufficiency preferred.
- Language: preferably bilingual in the county’s threshold languages.

- Qualified candidates with arrest and conviction records will be considered for employment.
- Must be willing to complete a background check and livescan.
