PRN Rotating Shift

We provide a broad array of medical, surgical, and behavioral health care serving Baltimore City and the region.   • Assess assigned patients, integrating psychosocial assessments within the multidisciplinary team. Collaborate to identify risks to recovery and develop care plans to determine appropriate levels of care upon discharge from the acute setting.
  • Develop and coordinate Discharge Plan A and alternative Plan B, including documentation in the medical record.
  • Collaborate with physicians, nursing, social work, and other disciplines, departments, payers, and agencies to eliminate barriers to efficient care delivery in the appropriate setting.
  • Facilitate timely transition planning.
  • Lead efforts in managing inpatient length of stay and reducing readmissions organization-wide.
Qualifications

III. Education and Experience

  • Valid licensure as a Registered Nurse or equivalent healthcare license in Maryland required.
  • BSN or equivalent healthcare degree required.
  • Minimum of 2 years of case management experience required.
  • Current and active specialty certification preferred.

IV. Knowledge, Skills, and Abilities

  • Knowledge of managed care, discharge planning, and case management required.
  • Effective verbal and written communication skills required.
  • Strong communication, confidence, and experience working with physicians required.
  • Excellent analytical and team-building skills, with the ability to prioritize and work independently required.
  • Ability to collaborate effectively across disciplines required.
  • Proficiency with hospital/case management software programs required.
  • Knowledge of growth and development principles across the lifespan and ability to assess data relevant to patient-specific needs required.

Ensure patient safety in all job functions and participate in safety initiatives.

  • Take action to correct observed safety risks.
  • Report adverse events and near misses.
  • Identify and communicate potential risks in processes, procedures, or devices.
