The Case Manager II is responsible for coordination of care and service delivery for an assigned group of patients and promoting inter-dependent communication and collaboration with the patient, caregiver, physician, and other members of the health care management team. The Case Manager will assess, implement, coordinate, monitor, and evaluate available services and resources to promote quality, cost-effective outcomes appropriate to the patient's needs and resources. Education/Formal Training Requirements
  • Bachelor's Degree Nursing - RN

  • 1-3 years Case Management

Licenses and Certifications Requirements
  • Accredited Case Manager (ACM) American Case Management Association Certification
  • Case Manager - The Commission for Case Manager Certification
  • Registered Nurse Arkansas - Arkansas State Board of Nursing
  • Registered Nurse Mississippi - Mississippi Board of Nursing
  • Registered Nurse Tennessee - Tennessee Board of Nursing
