The role requires a Bachelor's degree in a human services field and experience in case management; driving may be required.

Qualifications

  • Bachelor's degree in a human services field, or equivalent combination of training and experience.
  • Experience in a human services field related to the area of assignment, to include work in case management.
  • Some assignments also require specific experience working with various types of client populations.
  • One (1) year of experience in a human services field related to adults.

Responsibilities

  • Completes a face-to-face assessment to determine functional limitations, eligibility for service, progress and need areas at least once every 90 days.
  • Develops an Individualized Service Plan (ISP) based on assessment information in collaboration with the consumer and, if appropriate, family or significant others.
  • Links to services, resources, and supports specified in the ISP.
  • Assist the individual directly by obtaining the resources needed; advocate for the consumer to procure needed resources or services.
  • Coordinates services and service planning with other providers within NCSB and with other agencies, and private providers, such as psychiatric hospitals, counselors, vocational programs, doctors, etc., and obtains clinical documentation from other providers to ensure continuity of care.
  • Makes contact with family or significant others to promote implementation of ISP and community stability.
  • Monitors progress, service delivery, physical and mental stability, and consumer satisfaction by providing monthly service to all individuals on assigned caseload.
