Job summary

AltaMed Health Services Corp. seeks a Case Manager II to deliver comprehensive case management and outreach to a diverse youth population in Los Angeles. You will interview, counsel, assess, organize caseload, mentor, and support program goals.

You will supervise and mentor other department members and volunteers, ensuring quality care and adherence to the program's Scope of Work. Bilingual Spanish/English is preferred.

Qualifications

• Bachelor's degree in Social Services, Sociology, Human Services, Counseling, or related field required.
• Minimum 3 years of experience working with adolescents in under-served communities or public assistance programs.
• Bilingual Spanish/English preferred due to the population served.
• A valid BLS certification or higher, following AHA/Red Cross guidelines.

Job Overview

The Case Manager II is to provide comprehensive case management and outreach services to a diverse youth population. The Case Manager follows the program's Scope of Work activities to achieve the program goals through interviewing, counseling, assessment, caseload organization, mentoring, and management. The Case Manager also provides work oversight and mentoring to other members of the department and/or volunteers as needed.

Minimum Requirements

• Bachelor's degree in Social Services, Sociology, Human Services, Counseling, or related field required.
• Minimum 3 years of experience required working with adolescents in under-served communities or public assistance programs.
• Bilingual Spanish/English preferred due to the population served.
• A minimum requirement of a valid BLS certification or higher, following the American Heart Association (AHA) or the American Red Cross guidelines.

Compensation

$33.55 - $41.93 hourly
