The Case Manager is responsible for providing case management services to individuals of all ages with a primary diagnosis of intellectual or developmental disability. Responsibilities include:

1. Assessing service needs
2. Developing access plans to services
3. Liaising with clients, families, programs, and service providers
4. Monitoring client service needs
5. Advocacy
6. Consultation and education to clients, families, and the community

The Case Manager reports to the Director III of Greene County Clinic and is expected to work with initiative and independent judgment, following standard practices and guidance from the supervisor. Duties are performed in accordance with professional ethics and established policies.

Qualifications include:

• Bachelor’s Degree in Human Services or related field
• At least 1 year of experience in human services or equivalent education and experience
• Experience in developmental disabilities preferred

Must possess a valid Virginia Driver’s License and an acceptable driving record. For personal vehicle use, valid insurance is required.
