Job Summary:
The Providence Center Case Manager ICT Pawtucket Health Home Team will coordinate services for consumers to minimize stressors and facilitate the reduction of psychiatric and substance abuse disorder symptoms. The role involves encouraging consumers to attain the highest possible levels of independence.
Responsibilities:
- Coordinate a variety of services.
- Provide support counseling, problem solving, contracts, and limit setting.
- Establish attainable goals with the consumer.
- Participate as a member of a multidisciplinary team in developing and implementing therapeutic services, including working with substance abusers.
- Present verbal case summaries as required.
- Accompany consumers to appointments with agencies, clinics, doctors, etc.
- Make assessments and provide crisis interventions and risk management as needed.
- Assess unmet needs (psychological, substance use, physical, housing, psychiatric).
- Identify, assess, and respond to crisis situations.
- Teach consumers adult daily living skills.
- Maintain a clean, healthy, and safe environment.
- Initiate treatment and advocate within the network of care.
- Schedule appointments within The Center and with outside agencies.
- Assist with benefits acquisition (SSI, food stamps, advocates office, medical services).
- Complete necessary paperwork and maintain accurate records.
- Identify, obtain, and maintain housing and household goods.
- Negotiate with landlords, courts, families, and the treatment team on behalf of consumers.
- Attend court and mental health reviews.
- Transport consumers using own vehicle or Center van.
- Ensure confidentiality and adhere to documentation standards.
- Maintain cooperative relationships with staff, consumers, and community agencies.
- Attend trainings, case presentations, and conferences.
- Perform other duties as assigned.
Requirements:
- Associate's degree required; Bachelor's degree in Human Services, Social Work, or related field preferred.
- Experience in Human Services preferred.
- Valid driver's license, insurance, and registration required.
- Ability to interpret policies and procedures.
- Effective communication skills, both written and verbal.
- Ability to collaborate with community agencies.
