Case Manager I (RN/MSW) - Medicare/DSNP team - Seattle area

This position is remote and targeted at individuals residing in the Seattle area, knowledgeable about local resources. The role involves assisting Medicare and Dual Plan members through education, care coordination, and connecting to community resources.

Responsibilities:
  • Deliver operational case management and coordination services for members with varying health conditions.
  • Improve members' quality of life and promote cost-effective outcomes using internal and community resources.
Qualifications:
  • Bachelor's in nursing or master's in social work/behavioral health (preferred).
  • Unrestricted license as an RN, LSWAA/LSWAIC, or LMHC/LMHP/LMFT in Washington State (required).
  • Minimum one year of case management, home health, discharge planning, or related experience.
  • Experience in outpatient or inpatient clinical settings for at least three years.
  • Proficiency with Microsoft Office.
  • Case Management Certification (preferred).
  • Bilingual abilities (preferred).
