Job summary

Oklahoma Department Of Human Services is seeking a Case Manager I/II to assess needs, coordinate with individuals and agencies, and support emergency services in Oklahoma. Training through the Case Manager Academy is required for new employees.

Level I offers $40,000.87+ and Level II offers $43,000.94+ with full state employee benefits. Travel and a valid driver’s license are required.

Qualifications

  • Bachelor’s degree in a human services field and one year of professional experience OR nursing license and one year of experience.
  • Bachelor’s degree in a human services field includes related disciplines.
  • Two years of professional experience for Case Manager II OR nursing license with two years experience.

Responsibilities

  • Respond to requests for emergency services and coordinate with individuals and agencies to develop an emergency plan.
  • Review eligibility documentation and submit cases for eligibility determinations.
  • Ensure cases are accurately coded in CCM, IMS, and WAM.

Case Manager I/II

Annual Salary:

  • Level I H21A- $ 40,000.87+ Full State Employee Benefits
  • Level II H21B - $43,000.94+ Full State Employee Benefits

Travel is extensive.Must possess a valid driver's license and must maintain required car insurance.

Minimum Qualifications:

All New Employees will complete the Case Manager Academy Training Program

Case Manager - Level I
  • Education and Experience requirements at this level consist of a bachelor’s degree in a human services field* and one (1) year of professional experience working directly with individuals with intellectual and/or developmental disabilities or in social work, case management, special education, psychology, counseling, vocational rehabilitation, physical therapy, occupational therapy, speech therapy, nursing or a closely related field;
  • OR possession of a valid permanent Oklahoma license as approved by the Oklahoma Board of Nursing to practice professional nursing and one year of experience working directly with individuals with intellectual and/or developmental disabilities. Combination of education and experience.

*For purposes of the Case Manager job family “a bachelor’s degree in a human services field includes any degree from an accredited college or university except for a degree in a physical, natural or biological science or mathematics.

Case Manager - Level II
  • Education and Experience requirements at this level consist of a bachelor’s degree in a human services field* and two (2) years of professional experience working directly with individuals with intellectual and/or developmental disabilities or in social work, case management, special education, psychology, counseling, vocational rehabilitation, physical therapy, occupational therapy, speech therapy, nursing or a closely related field;
  • OR possession of a valid permanent Oklahoma license as approved by the Oklahoma Board of Nursing to practice professional nursing and two years of experience working directly with individuals with intellectual and/or developmental disabilities. Combination of education and experience.

Job Responsibilities:
  • The intake CM responds to requests for emergency services, provides community referrals, and coordinates with individuals and other agencies to develop a plan to resolve the emergency
  • . The intake CM receives the required eligibility documentation, reviews it for accuracy, and electronically submits the case to OHCA for an eligibility determination from OHCA.
  • The intake CM ensures that cases are accurately coded in CCM, IMS, and WAM.
