Job summary

The Oklahoma Department of Human Services in Ponca City, Oklahoma, is hiring for a Case Manager I/II. The position offers annual salary levels: Level I $40,000.87+ with benefits; Level II $43,000.94+ with benefits.

Travel is extensive and requires a valid driver's license and car insurance. Minimum qualifications include a bachelor's degree in a human services field and one year of relevant experience; or a permanent Oklahoma nursing license plus one year of experience.

Qualifications

• Bachelor’s degree in a human services field and one year of relevant experience required.
• Or possession of a permanent Oklahoma nursing license and one year of relevant experience.
• For purposes of the Case Manager job family, a bachelor’s degree in a human services field includes any degree from an accredited college or university except for a degree in physical, natural or biological science or mathematics.

Responsibilities

• Under the direction of a case management supervisor, provide services to individuals with intellectual and/or developmental disabilities living in their homes or community settings; obtain in-depth social histories and coordinate resources.
• Meet compliance with Federal Home and Community Based Waiver regulations, state, and policy guidelines.

Case Manager I/II

Annual Salary:

• Level I H21A- $ 40,000.87+ Full State Employee Benefits
• Level II H21B - $43,000.94+ Full State Employee Benefits

Minimum Qualifications:

All New Employees will complete the Case Manager Academy Training Program

Case Manager - Level I

• Education and Experience requirements at this level consist of a bachelor’s degree in a human services field* and one (1) year of professional experience working directly with individuals with intellectual and/or developmental disabilities or in social work, case management, special education, psychology, counseling, vocational rehabilitation, physical therapy, occupational therapy, speech therapy, nursing or a closely related field;
• OR possession of a valid permanent Oklahoma license as approved by the Oklahoma Board of Nursing to practice professional nursing and one year of experience working directly with individuals with intellectual and/or developmental disabilities. Combination of education and experience.

Case Manager - Level II

• Education and Experience requirements at this level consist of a bachelor’s degree in a human services field* and two (2) years of professional experience working directly with individuals with intellectual and/or developmental disabilities or in social work, case management, special education, psychology, counseling, vocational rehabilitation, physical therapy, occupational therapy, speech therapy, nursing or a closely related field;
• OR possession of a valid permanent Oklahoma license as approved by the Oklahoma Board of Nursing to practice professional nursing and two years of experience working directly with individuals with intellectual and/or developmental disabilities. Combination of education and experience.

Job Responsibilities:

• Under the direction of a case management supervisor, the position provides services to individuals with intellectual and/or developmental disabilities living in their own homes or community living alternatives; obtains in-depth social histories, develops and assures community and/or agency resources to meet the consumer's needs; serves as QIDP and provides monthly monitoring of the program.
• This position is required to meet compliance with Federal Home and Community Based Waiver regulations, state, and policy guidelines.
