Qualifications

• Must complete Case Manager Academy Training Program.
• One year of professional experience with applicable fields for Level I.
• Two years of professional experience with applicable fields for Level II.

Responsibilities

• Provide services to individuals with disabilities living at home or community alternatives.
• Develop and manage community resources to meet consumer needs.
• Monitor program compliance with regulations.
