The Integrated Care Center Clinic is a federally qualified health center that provides primary care, behavioral health, addiction treatment, psychiatry, dental care, and more. The role involves managing the Enhanced Case Management (ECM) program, providing intensive, client-centered support to help clients achieve stability, health, and self-sufficiency. The Case Manager will maintain caseloads, link clients to services, and coordinate care across providers, including outreach in community settings.

Key Responsibilities:

Client Care and Coordination:

  • Organize patient care activities, share information with the multidisciplinary team, and implement the Managed Care Plan.
  • Identify and enroll eligible individuals from prisons, jails, hospitals, and other locations.
  • Engage clients through various strategies, including in-person visits, outreach, and communication via mail, email, and phone.
  • Maintain communication with all care team members and ensure continuous, integrated care.
  • Support client engagement through appointment scheduling, reminders, transportation, and barrier mitigation.
  • Identify care gaps and coordinate services including primary care, mental health, housing, and social services.
  • Assist with intake assessments and monitor progress toward care plan goals.
  • Connect clients to benefits and community resources, including housing and employment services.

Clinical Documentation and Administrative Tasks:

  • Complete progress notes within 24 hours of service.
  • Prepare progress letters and court reports as needed.
  • Participate in chart audits, supervision, meetings, and training.

Training, Supervision, and Quality Improvement:

  • Participate in case conferences, team meetings, and training sessions.
  • Communicate effectively within the behavioral health team.
  • Complete required training and courses promptly.

Field-Based Responsibilities:

This position requires frequent travel to meet clients in various community locations and to utilize a provided office space for case management and training.

Qualifications:

Education, Certification, or Licensure:

  • High school diploma or equivalent for Case Manager I.
  • Bachelor’s degree in a human services-related field for Case Manager II.
  • At least two years of experience in human services or substance use treatment preferred.
  • Current First Aid and CPR certification.
  • Valid California driver’s license and insured transportation.
  • Experience with clients in acute withdrawal and trauma-informed care preferred.
