Job summary

Catholic Charities Brooklyn and Queens, Inc. is seeking a CRED Case Manager I to deliver professional intervention and comprehensive support to clients in Brooklyn and Queens. You will engage, assess, assist with applications, connect to services, and follow up to ensure goals are met.

Under the Program Manager, you will work with a multidisciplinary team, provide coaching, and help participants access training, internships, or employment opportunities across the five boroughs.

Qualifications

• Bachelor's degree in social work, Human Services, Counseling, or a related field (or AA with two years' related field experience).
• Proven experience in case management, counseling, or a related field with vulnerable populations.
• Available to work in person 5 days a week; flexible hours including weekends/evenings; able to travel across the five boroughs.

Responsibilities

• Provide case management, assessment and intake to determine eligibility and connect to services.
• Conduct initial assessments to identify strengths, needs and goals with the team.
• Develop individualized case plans with participants outlining goals and strategies.
• Provide ongoing support, address barriers and adjust plans.

For over 125 years, Catholic Charities Brooklyn and Queens has been providing quality social services to the neighborhoods of Brooklyn and Queens and currently offers 160-plus programs and services for children, youth, adults, seniors, and those struggling with mental illness.

STATEMENT OF THE JOB

Under the direction of the Program Manager, the CRED Case Manager I renders professional intervention to clients in need of assistance through comprehensive support services (engagements, assessment, intake, application assistance, connection to services and resources to meet their goals, retention and follow-up).

• Provide case management services, assessment and intake of participants to determine eligibility for services, including responding to telephone and walk-in inquires, conduct pre-screenings and provide referrals for those not eligible.
• Conduct initial assessment to identify participant's strength, need and goals in collaboration with the mental health counselor, job developer and outreach/retention specialist.
• Develop individualized case plans with participants, outlining goals, objectives and strategies for success.
• Provide ongoing support and guidance to participants, addressing barriers and challenges
• Facilitate connections to community resources, services, and support networks. Refer participants to relevant resources, services, and opportunities.
• Monitor participant progress, adjusting case plans as needed. Conduct field visits to training sites, participants internships and/or employment.
• Offer weekly one-on-one coaching and counseling to participants, focusing on goal setting, problem-solving, and decision-making skills in the field and office.

QUALIFICATIONS:

• Bachelor's degree in social work, Human Services, Counseling, or a related field or AA degree in social work, Human Services, Counseling, or a related field with two years' related field experience in City systems such as justice, public assistance, child welfare, housing or other relevant social service setting providing direct services.
• Proven experience in case management, counseling, or a related field, working with the target population or vulnerable populations.
• Must be available to work in person 5 days a week.
• Ability to work flexible hours, including weekends, evenings according to the needs of program operation.
• Able to conduct home/field visits and travel throughout the five boroughs as needed.
• Ability to utilize public transportation, climb stairs, walk.
• Frequent sitting, standing, & walking.
