Job summary

Oklahoma Department Of Human Services in Duncan, Oklahoma, seeks a Case Manager I/II to support individuals with intellectual and developmental disabilities in home and community settings. Under supervision, you will gather histories and coordinate resources to meet consumer needs as a QIDP with monthly monitoring.

Two levels exist: Level I requires a bachelor’s degree in a human services field and 1 year of related experience or a nursing license with 1 year of related experience; Level II

Qualifications

Bachelor’s degree in a human services field completed from an accredited college or university (excluding physical, natural or biological sciences or mathematics).
Permanent Oklahoma nursing license with one year of direct experience working with individuals with intellectual and/or developmental disabilities or related fields.

Responsibilities

Under supervision, provides services to individuals with intellectual and/or developmental disabilities living in their own homes or community living settings; obtains social histories, develops resources to meet consumer needs; serves as QIDP and conducts monthly monitoring.
Must meet compliance with Federal Home and Community Based Waiver regulations, state and policy guidelines.

Education

Bachelor’s degree in a human services field

Permanent Oklahoma nursing license (as approved by Oklahoma Board of Nursing)
