Position Summary

Provides case management services to patients assigned to an individual caseload.

Salary Range

$25.75 - $29.50 per hour DOE

Categories of Duties
General Duties:
  1. Conduct oneself professionally in dress, language, attitude, and behavior.
  2. Support the philosophy and objectives of the Agency.
  3. Adhere to professional ethics policies.
  4. Inform patients about other services offered by Tarzana Treatment Centers, Inc. (TTC).
  5. Attend in-service trainings and educational workshops.
  6. Participate in staff meetings.
  7. Communicate effectively with team members regarding patient care.
  8. Collaborate with multidisciplinary team members to develop comprehensive treatment plans.
  9. Assist with employment/vocational referrals and counseling.
  10. Provide outreach and follow-up services to patients.
  11. Facilitate discharge planning, including housing, employment, and aftercare.
  12. Coordinate with internal and external entities to resolve legal, financial, housing, and medical issues.
  13. Maintain detailed progress notes in patients' medical records.
  14. Perform other duties as assigned.
Experience Requirements:

For AOD Programs: Enrollment in an academic Addiction Studies program, registration with CCAPP, CAADE, or CADTP, and 1 year experience in behavioral healthcare.

For other programs: 1 year experience in behavioral healthcare (HIV programs) and DVSS Certification for DV-specific staff.

Licenses/Certificates:

For AOD programs: Certification with a DHCS-approved state counselor certification agency.
