Starting Pay Range -

CM I - Bachelors: $22.62 - $24.87 / hour

CM II- Masters: $26.00 – $29.96 / hour

Position Description:

Case Managers work with consumers who have mental illness. They are part of a multi-service treatment team and often serve as the point person for the treatment team, helping to facilitate and coordinate services within and outside of CRMHS. Case Managers provide education about mental illness, assist consumers with achieving their goals, and help with skill building to promote independence and recovery. They work with individuals, family members, and significant members of social and support networks, leading a variety of psycho-educational groups. Case Managers may work with their clients in the clinic, in the home, and in the community.

This position is a housing case manager role that involves a high level of coordination with community partners, such as the VHA and Council for the Homeless. The case manager will manage the referral process for housing options through the VHA and Key properties, such as Central Park Place (SRO), Rhododendron Place, and Teeny Creek.

The Housing Specialist will have two primary scopes of work: 1) home and community-based supports to eligible individuals, including coaching, advocacy, information and referral, linking and coordinating, and ongoing supports; and 2) identifying housing opportunities in the community through outreach to private market and scattered-site landlords.

Essential Job Functions:

  • Work collaboratively with each client to develop a unique treatment plan based on the individual’s recovery goals.
  • The treatment plan addresses the following life domains: Mental Health; Alcohol/Drug Issues; Activities of Daily Living; Work/School; Family/Natural Support; Income/Finances; Social/Friends/Fun; Legal; Medical and Dental; Housing/Shelter; Cultural/Spiritual; Other.
  • Work on site at Rhododendron Place approximately 2 days a week.
  • Support clients already in CRMHS housing and housing retention.
  • Maintain all required paperwork/charting requirements as established by agency contracts.
  • Achieve and maintain agency standard productivity requirements.
  • Tenant-Landlord Mediation; Prepare landlords for high acuity clients entering their units.
  • Provide ongoing tenancy supports as needed to ensure individuals remain stably housed.
  • Conduct functional needs assessments to identify participant tenancy preferences.
  • Develop individualized community integration plans based on the functional needs assessment.
  • Work with participants to develop short and long-term measurable goals.
  • Provide service-planning support and participate in person-centered plan meetings.
  • Coordinate and link participants to community support services, including those designed to obtain and maintain independent living.
  • Provide entitlement assistance, including obtaining documentation, navigating and monitoring the application process, and coordinating with the entitlement agency.
  • Provide supports to assist participants in developing independent living skills.
  • Coordinate with participants to review, update, and modify their housing support and crisis plan.
  • Connect participants to resources that will assist in being good tenants.
  • Work with private market and scattered-site landlords, and grant-funded opportunities to grow and foster housing placement opportunities.

Non-essential Job Functions:

  • Support agency mission, goals, and objectives.
  • Attend staff and planning meetings as scheduled.
  • Maintain communication and cooperative working relationships both within the agency and community, while maintaining a high degree of confidentiality.
  • Other duties as assigned.

Education, Licensing and Experience:

  • Bachelor’s degree in Mental Health, Social Work, Psychology, or Human Services.
  • Approximately 1-2 years of experience in the social service field with mental illness or chemical dependency preferred.
  • Approximately 1-2 years of experience working in the housing realm preferred.
