Job summary

Samaritan Daytop Village seeks a dedicated Case Manager in Unionport, Bronx to interview clients, assess needs, and develop living plans. You will coordinate housing referrals, track progress, and advocate to remove barriers to stability.

Build trust with diverse clients, maintain confidential records, and partner with housing, healthcare, education, and legal services to support outcomes. This is a full-time, non-exempt position with full benefits.

Qualifications

• High School Diploma or equivalent; associate or bachelor's in human services preferred.
• Minimum 1 year in case management or social services with homelessness experience.
• Strong interpersonal and communication skills.
• Ability to maintain records and meet reporting deadlines.
• Commitment to client confidentiality and ethics.
• Familiarity with NYC housing, healthcare, legal, childcare, and education resources.
• Organized, detail-oriented, and culturally sensitive.
• Ability to collaborate with external partners and advocate for clients.

Responsibilities

• Build relationships with clients while maintaining confidentiality.
• Review eligibility documents and explore alternative housing options.
• Create and manage client files and records.
• Conduct intake interviews and psychosocial assessments.
• Develop and update Independent Living Plans with goals.
• Refer clients to housing and community services.
• Support clients to overcome barriers and advocate for themselves.
• Track progress and document updates through regular contact.
• Follow up with clients and agencies to monitor outcomes.
• Provide data for weekly, monthly, quarterly and annual reports.
• Liaise with external organizations on education, healthcare, housing and legal matters.

Education

High School Diploma or equivalent

Associate or Bachelor’s Degree in Human Services or related field
