Position Overview

The Case Manager | Housing Specialist plays a dual role in APF’s Housing Program, supporting individuals and families who are experiencing or at risk of housing insecurity. This position provides strength-based case management, advocates for client needs, and makes community-based referrals. As a Housing Specialist, you will also work to increase housing access by building relationships with landlords, educating clients on their rights, and offering personalized housing search support.

Key Responsibilities
  • Conduct outreach and assessments to identify clients' housing and supportive service needs
  • Develop individualized care plans and provide regular, strength-based case management
  • Advocate for clients in navigating barriers such as housing, food insecurity, and transportation
  • Coordinate with internal staff and external community partners for holistic care and referrals
  • Build and maintain relationships with landlords and housing providers to expand options for participants
  • Facilitate housing search counseling, workshops, and one-on-one navigation sessions
  • Educate clients on tenant rights, fair housing laws, and housing readiness
  • Maintain accurate case documentation in the Unite Us platform and other systems
  • Participate in team meetings, local outreach, training, and professional development opportunities
Qualifications

Required
  • Bachelor’s degree in Psychology, Counseling, Rehabilitation Counseling, or a related field
  • OR Master’s degree in Social Work, Psychology, Counseling, Vocational Rehabilitation, or related field
  • Minimum of 2 years of related experience
  • Knowledge of community resources and services in Orange County
  • Ability to work with a wide range of disabilities and diverse populations
  • Strong interpersonal and communication skills—both written and verbal
  • Comfortable in emotionally charged or high-pressure situations
  • Proficiency in using documentation platforms (training provided for Unite Us)
Preferred
  • Experience or interest in mindfulness and meditation practices

The pay range for this role is:

49,920 - 58,240 USD per year (Long Beach)
