Job Overview

Reporting to the Program Manager, the Case Manager delivers high‑quality, client‑centered case management services for individuals and families experiencing homelessness. You will manage a caseload of 15–20 participants, serving as their central point of contact.

Key Responsibilities

• Conduct initial and quarterly psychosocial assessments.
• Collaborate with participants on individualized service plans and housing stability goals.
• Build trusted, professional relationships with clients.
• Provide outreach, housing navigation, and ongoing support.
• Facilitate referrals to healthcare, mental health, substance use, and community services.
• Support clients in obtaining employment, benefits, and education opportunities.
• Accompany clients to key appointments as needed.
• Use a harm reduction, strength‑based, and client‑focused approach.
