Responsibilities

  • Provide case management services to support client stability and recovery.
  • Assist clients with identifying and accessing community resources, including housing, employment, transportation, healthcare, benefits, and recovery supports.

  • Maintain a safe, respectful, and treatment-focused group environment.
  • Encourage client participation, engagement, accountability, and progress.
  • Assist clients in developing practical coping, life, and recovery skills.
  • Participate in treatment team meetings and communicate relevant client updates.
  • Collaborate with therapists, clinical leadership, and other members of the treatment team.
  • Assist with referrals, continuity of care, and discharge planning.
  • Recognize changes in client functioning, behavior, mood, or safety and communicate concerns appropriately.
  • Complete accurate, timely, and objective case management and group documentation.
  • Maintain confidentiality and comply with HIPAA and organizational policies.
  • Maintain professional boundaries with all clients.
  • Support a trauma-informed, person-centered, and recovery-oriented treatment environment.

Case Manager / Group Facilitator
About the Role

Divine Intervention Recovery Center is a dual-diagnosis treatment center serving adult men and women experiencing substance use disorders, mental health conditions, and co-occurring disorders. We are seeking a compassionate, dependable, and motivated Case Manager/Group Facilitator to join our treatment team with a minimum requirement of a CDCA.

This position combines case management, group facilitation, client engagement, resource coordination, and collaboration with the clinical team. The ideal candidate is someone who can build professional rapport with clients while maintaining appropriate boundaries, facilitate structured and meaningful groups, and help clients overcome practical barriers that may interfere with treatment and recovery.

Key Responsibilities
  • Provide case management services to support client stability and recovery.
