Job summary

CareStar, Inc. invites a Case Manager to support the Elderly and Disabled Waiver program in Muscogee County and surrounding areas. The role combines remote work with field visits to assess client needs and connect them with services.

Ideal candidates have nursing licensure and at least two years' experience in geriatric care or long-term services, plus strong assessment and care coordination skills. Travel and a flexible schedule are required.

Qualifications

• Two years’ RN or LPN experience in geriatrics, community health, long-term care, or chronic diseases.
• Experience completing assessments and developing/monitoring service plans.
• Strong organizational and communication skills.
• Experience in care management/coordination preferred.
• Willingness to travel and have valid driver's license and car insurance.

Key Responsibilities

• Adheres to the CareStar Rule in performance of job responsibilities.

• Identify, coordinate and oversee the delivery of services to enhance the health, safety, and general well-being of participants to assist the participants goals toward maximum independence.

• Understands and complies with CareStar Policies and Procedures.

• Maintains confidentiality as related to patient information. Any disclosures of confidential information made unlawfully outside the proper course of duty will be treated as a serious disciplinary offense.

• Follows the Acceptable Use Policy while using any information systems owned or controlled by CareStar, Inc.

Minimum Qualifications

• Two years’ experience as a Licensed Practical Nurse or Registered Professional Nurse in one of the following areas: Geriatric nursing, community health, long-term care, and/or chronic diseases of adults.

• Experience in completing assessments, service plan development and monitoring.

• Effective organizational and communication skills

• Experience providing care management/coordination preferred.

• Willingness and ability to work flexible schedule to meet needs of families including reliable transportation, valid driver’s license and car insurance as required by state law, and the ability to travel.
