Looking for RNs in the Milwaukee, Sheboygan & Central WI areas with some Hospice experience!

Full Time RN Schedule (Milwaukee areas): This will be an hourly position with wage up to $45/hr (based on experience); Monday-Friday 1st shift.

Full Time RN Schedule (Sheboygan areas): This will be an hourly position with wage up to $45/hr (based on experience); Monday-Friday 1st shift. This position is eligible for a $5,000 sign-on bonus.

Full Time RN Schedule (Central WI areas): We are expanding into Central WI and need to fill up our team! This will be an hourly position with wage up to $45/hr (based on experience); Monday-Friday 1st shift.

Summary/Objective:

Responsible for planning, organizing, developing, and directing the nursing care of the hospice patient in accordance with current federal, state, and local standards, guidelines, and regulations.

Essential Functions:
  • Assisting the admission nurse in the coordination of timely admissions
  • Provide hands-on clinical nursing, interacting with patients, family, physicians, Interdisciplinary Team members, and other health care professionals involved in the patient’s care.
  • Assess and coordinate a pain/symptom management program with the participation of family and patient, attending physician, and the Interdisciplinary Team, utilizing pharmacological and non-pharmacological methods of pain/symptom control to enhance quality of life.
  • Assist in facilitating freedom of choice, relief of suffering, and death with dignity for patients facing end-of-life issues
  • Educate patient and family on the disease process and expectations related to disease progression
  • Provide support and education for families based on the hospice philosophy of care, including physical, psychological, spiritual, and financial comfort.
  • Participate in Interdisciplinary Team meetings and update the Plan of Care (POC) for assigned patients based on identified problems and needs
  • Ensure patient care is delivered at the appropriate level of care
  • Manage medication profiles in coordination with PCP and medical director to prevent polypharmacy and ensure optimal symptom management
  • Ensure all services provided are in accordance with the patient’s Plan of Care
  • Coordinate services including communication and assignments for visiting nurses, home health aides, chaplains, social workers, volunteers, and ancillary services, as well as ordering medications and DME
  • Provide consultation and supervision to CNAs and LPNs involved in patient care
  • Participate in performance evaluations of clinical team members involved with your caseload
  • Assist with orientation of clinical staff
  • Ensure cost-effective practices
  • Assist with QAPI activities
  • Work as a team member and uphold the Hospice Code of Ethics
  • Provide patient care management
  • Utilize medication formulary for symptom management
  • Triage scheduling and symptom management calls from IDT members
  • Report to the Clinical Director regarding patient condition changes
Work Environment/Physical Requirements:

Position involves physical activities such as standing, lifting, bending, stooping, pushing, pulling, and twisting. Must have a valid driver’s license, a reliable vehicle, and be insured. Must adapt to weather conditions and travel as needed.

Required Education and Experience:
  • Registered Nurse licensed in Wisconsin, graduated from an accredited program, in good standing.
Preferred Education and Experience:
  • 1-2 years of nursing experience in acute care, and 1 year in home health, hospice, or related fields.
  • Interest and aptitude for hospice philosophy and comfort care.
  • Ability to work cooperatively with interdisciplinary team members.
