Job summary

CHS in Phoenix, AZ is seeking qualified behavioral health professionals to support crisis care services. The role includes stabilizing individuals in crisis, facilitating admissions, treatment planning, and discharge planning, with collaboration from an Interdisciplinary Treatment Team.

Expect full-time work with shift differentials and responsibilities including accurate record-keeping and adherence to Title 36 processes and local funding avenues in Maricopa County.

Qualifications

• Associate’s degree in behavioral health or a related field.
• At least 2 years of behavioral health experience.
• Compliance with the Company’s mandatory vaccination policy.

Responsibilities

• Coordinate with an Interdisciplinary Treatment Team to plan and discharge care with families, local providers, and community partners.
• Collaborate to develop individualized Treatment Plans for brief intervention.
• Gather collateral to assess immediate treatment and discharge needs.
• Coordinate services to meet patient needs at discharge and address gaps in care.

• +$3/hr – Friday-Sunday Nights
• +$2/hr – Saturday-Sunday Days

What You’ll Do

• Manage care of individuals in crisis, stabilizing them from acute episodes and facilitating admission intakes, treatment planning, discharge planning, collateral gathering, legal assistance, and initiation of personalized care plans.
• Work closely with an Interdisciplinary Treatment Team to coordinate care and discharge planning with families, local providers, and community partners, carrying a caseload and acting as an ambassador to community partners.
• Collaborate with the Crisis Therapist to develop and implement individualized person‑centered Treatment Plans appropriate for brief intervention.
• Gather collateral from the individual and involved parties to provide a thorough assessment of immediate treatment and discharge needs.
• Actively coordinate care to ensure services align with facility staff, identify and address gaps in service needs, and recommend solutions to meet patients’ needs upon discharge.
• Engage, monitor, and communicate with the individual with the Interdisciplinary team to maintain safety while meeting individual needs.
• Maintain clinic records, documenting activities, obtaining necessary signatures, and recording services in the medical record.
• Maintain knowledge of the Title 36 Involuntary Commitment process, monitor petitions for timeliness, ensure forms are complete, and attend COE hearings when subpoenaed.
• Maintain knowledge of the Maricopa County RBHA system and potential funding sources.
• Participate in individual and group supervision, ongoing education, training, and performance assessments as required, and perform additional duties as assigned.

What You’ll Bring

It Would Be Great if You Had

• At least 4 years of behavioral health experience
• Bachelor’s degree in behavioral health or a related field
• Knowledge and experience with the AZ Title‑36 legal process
