Alcott is a leading nonprofit agency in Los Angeles County seeking a compassionate Case Manager to join our Full-Service Partnership program in Culver City. You will provide community-based mental health services to adults, coordinate treatment plans, housing placement, and care coordination in a field-based, full-time role.

The ideal candidate has a Bachelor's in a related field, experience with serious mental illness and DMH documentation, and bilingual English/Spanish is strongly encouraged.

• Experience with individuals living with serious mental illness preferred.

Responsibilities

• Provide community-based mental health services to adults in the Full-Service Partnership program.
• Develop and implement treatment plans in collaboration with clients and the multidisciplinary team.
• Assist clients with housing placement, care coordination, and connection to community resources.
• Conduct crisis intervention and participate in rotating on-call responsibilities.
• Maintain documentation per LACDMH and Medi-Cal standards and ensure service delivery is compliant.

Job Title: Case Manager
Department: Full-Service Partnership (FSP) Program
Reports To: Program Manager
FLSA Status: Non-Exempt
Work Setting: Primarily Field-Based
Schedule: Full Time, Monday through Friday, 8:30 am to 5:00 pm, including participation in rotating on-call coverage as assigned
Pay Range:$24.00 - $27.00 per hour, DOE

What You'll Do: The Impact You'll Make

As a Case Manager, you will provide community-based mental health services to adults participating in the Full-Service Partnership Program. You will support clients through treatment planning, housing placement, care coordination, crisis intervention, rehabilitation services, and community linkage.

Client Care and Service Coordination

• Coordinate medical follow-up appointments and services for clients with co-occurring medical and behavioral health conditions.
• Develop and implement treatment plans in collaboration with clients and multidisciplinary team members.
• Provide rehabilitation services, life skills training, and support for independent living.
• Assist clients with locating, securing, and maintaining stable housing placements.
• Connect clients to community resources and supportive services.

Documentation and Compliance

• Complete accurate, thorough, and timely documentation in accordance with Los Angeles County Department of Mental Health (LACDMH) and Medi-Cal standards.
• Maintain required productivity and service utilization expectations.
• Ensure documentation and service delivery remain compliant with agency and regulatory requirements.

Crisis Intervention and Community Support

• Respond to client crises and provide appropriate intervention and stabilization services.
• Participate in rotating on-call responsibilities, including after-hours crisis response when assigned.
• Conduct field-based outreach and engagement activities throughout Los Angeles County.

Team Collaboration and Professional Practice

• Work closely with multidisciplinary team members, service providers, and community partners.
• Communicate effectively with clients, families, providers, and stakeholders.
• Maintain professionalism, accountability, and client-centered service delivery.

What You'll Bring: Your Qualifications

Education and Experience

• Bachelor's degree in Psychology, Social Work, Counseling, Human Services, or a related field required.
• Experience working with individuals living with serious mental illness preferred.
• Experience with Los Angeles County Department of Mental Health documentation preferred.
• Experience in community mental health, supportive housing, case management, or related social service settings preferred.
• Bilingual English/Spanish candidates are strongly encouraged to apply.

• Valid California Driver License required.
• Clean driving record required.
• Must be insurable under Alcott's auto liability insurance policy.
• Live Scan clearance required.
• Evidence of a clear criminal record and eligibility to claim Medi-Cal.
