Centerstone seeks a Foster Care Case Manager to safeguard the welfare of foster children and coordinate services across families, schools and community providers. The role requires in-home visits, documentation in Centerstone EMR, and active liaison with DCS and foster families.

Provides for the safety, permanency and well‑being of foster care clients, serving as the primary link between Centerstone, DCS, foster families, biological families, schools, and services needed. Conducts in‑home visits and manages issues and concerns of foster children/families, provides linkage and referral to other services and supports to enhance family functioning. Compensation for this role is between $17.00-$18.77/hr. Mileage is also reimbursed.
