Full Time Case Manager

Applegate Recovery Services is seeking a dedicated and detail-oriented Case Manager to perform administrative tasks under supervision. The primary role is to support the treatment process by assisting therapists in meeting clients' needs. Additional duties may be assigned.

Essential Duties & Responsibilities:
  1. Act as a liaison between therapists, referral agencies, and clients' families. Arrange visits, coordinate transportation, and meet clients' needs.
  2. Maintain effective communication between families, referral sources, and Mahajan therapeutics.
  3. Attend court hearings as appropriate.
  4. Participate in treatment staffing with prepared reports on clients' responses to treatment.
  5. Facilitate individual and group CSP sessions as assigned.
  6. Attend supervisory sessions with a prepared agenda.
  7. Assist clients in achieving independence in managing basic needs, as identified by the client or guardians.
  8. Support the development of daily living skills, if identified as necessary.
Qualifications:
  • Experience working with adults with addiction preferred.
  • Must possess a CDCA certification.
  • Ability to interact professionally with colleagues, providers, patients, and families to promote teamwork.
  • Strong organizational, communication, and interpersonal skills.
  • Attention to detail, memory, reasoning, and ability to analyze and evaluate information.
  • Successful drug screening and background checks.
