What do we need?
As a Case Manager for Foster Care, you will provide case management and comprehensive community support services to children and their families.
More specifically, the candidate chosen for this role will:
- Maintain regular, ongoing contact with each child/family on caseload to ensure coordination and provision of needed services in compliance with the client’s plan of care. Contacts will occur face-to-face or via telehealth according to licensing regulations and identified need. Frequency/type of contact will be dictated by plan of care and program guidelines for which the client is enrolled.
- Deliver case management services aimed at ongoing assessment of the client’s need for medical, educational, social or other services and ensure coordination/provision of these services as indicated in the client’s plan of care.
- Engage in case management activities which may include making referrals for identified services, scheduling appointments, collaborating and maintaining communication with service providers, providing consultation and support to parents/legal guardians on an ongoing basis, and face-to-face monitoring/contacts with the client/family as often as necessary or at a minimum frequency as outlined in program guidelines.
- Provide comprehensive community support services for identified clients that will consist of activities needed to allow the client to live with maximum independence in the community through skills training as identified in the client’s plan of care.
- Initiate and maintain regular contact with other service providers involved with the child (e.g., school, therapists, social workers, medical professionals) and inform and involve other team members in the treatment planning process.
- Participate in and be responsible for the treatment team meetings and the development of the specific care plan.
- Complete required documentation within designated time frames and in compliance with reimbursement standards outlined by the Department of Medicaid Services or other payors.
- Actively plan for the effective transition of the client from services through ongoing discharge/termination planning and referral for services still in need of attention at the time of termination, if indicated.
- Provide supervision of therapeutic foster homes for clients in therapeutic foster care program as assigned. Ensure completion and obtain all required documentation by the foster family.
- Provide foster families with at least weekly contact by phone, two face-to-face contacts monthly, and one individual visit with the youth outside of the home monthly.
- Evaluate foster families annually for recertification, complete documentation, and support the recertification process.
- Assist families in implementing the in-home strategies specified in the treatment plan.
- Provide 24-hour “on-call” coverage, on a rotating basis, to families/clients in crisis situations.
- Assist prospective foster/adoptive families in completing certification/re-certification requirements, including participating in pre-service training as needed.
- Respond to referrals timely, document referral status, coordinate placements, and manage the process of placing children into therapeutic foster/adoptive homes.
Education and/or Experience:
- Bachelor's degree from an accredited university in social sciences or related field.
- One (1) year of full-time experience working with children in a human service setting.
Certificates, Licenses, & Registrations
- Successfully complete department-approved targeted case management training within six (6) months of employment and ongoing continuing education every three (3) years.
- Maintain current CPR and First Aid certifications.
- Possess a valid driver’s license, completion of safe driver's training, and an insurable driving record.
