Father Bill's & MainSpring is seeking an Upstream Prevention Case Manager in Southern Massachusetts to preserve tenancies and secure housing opportunities for FBMS participants. You will build relationships with landlords, researchers, state officials, and community partners while coordinating with internal teams to prevent homelessness.

The role emphasizes organization, problem solving, flexibility, and strong communication.

Qualifications

  • 2-4 years of housing or social work experience or related field.
  • CPR certification is required and provided by FBMS.
  • AED and First Aid may be required and provided by FBMS.
  • Valid driver’s license and daily access to a motor vehicle.

Responsibilities

  • Receive and respond to referrals from the Family Programs Manager, external non-profit and state entities, Regional Administrative internal providers such as Tenancy Preservation Program (TPP) and self-referrals by participants.
  • Complete an ongoing assessment of family’s immediate housing needs and assist in preserving tenancy or providing housing search assistance to obtain stable housing if required.
  • Provide updates to tenants, landlords, researchers, and stakeholders regarding progress and ongoing concerns, including weekly communication with researchers.
  • Network and maintain relationships with local landlords, internal and external stakeholders and community partners.
  • Collaborate with the Employment Specialist to ensure employment needs are addressed.
  • Assist families to identify community services, make referrals, and follow up; may participate in WrapAround Services.
  • Accompany and transport families to community services and apartment searches as appropriate.
  • Complete a Housing Action Plan with participants to address barriers to maintaining housing.
  • Create and encourage families to follow a budget to meet their needs.
  • Maintain written progress reports and documentation in client records and researcher databases.

Position Summary:

The Upstream Prevention Case Manager position is an innovative, full-time position responsible for working towards preserving existing tenancies and/or locating and securing housing opportunities for FBMS participants. Applicants need to work diligently to build and maintain strong, working relationships with local landlords, Harvard Kennedy School researchers, state officials and community partners as well as with the internal FBMS teams. The applicant would benefit from having an energetic, enthusiastic, creative, and outgoing mindset as well as strengths in organization, problem solving, and flexibility.

Essential Responsibilities:
  • Receive and respond to referrals fromthe Family Programs Manager, external non-profit and state entities(EOHLC), Regional Administrative internal providers such asTenancy Preservation Program (TPP) as well as self -referrals by participants.
  • Complete an ongoing assessment of family’simmediate housing needsand assist in preserving tenancy orproviding housing search assistance toobtain stable housing if required.
  • Provide tenant, landlord/property manager, researchers, and stakeholders withupdates regarding progress, and all identified ongoingconcerns asappropriate. This could include weeklycommunication with researchers.

  • Networking and maintaining relationships withlocal landlords, internal and external stakeholders and community partners.
  • Collaborate with the Employment Specialist to ensure employment needs are addressed.
  • Assist families toidentify community services, make appropriate referrals,and follow up. Mayserve as part ofa WrapAround Services team toassist participant(s) in maintaining housing.
  • Accompany and transport families tocommunity services and apartment searches as appropriate.
  • Complete a HousingAction Plan withparticipants toaddress barriers to maintaining housing
  • Create, implement and encourage families tofollow a reasonable budget to meet their needs.
  • Supervision, monitoring and reporting toresearchers ofparticipant’s progress with referralsand services for up to 90

days following participants stabilization into housing

  • Maintain writtenprogress reports and supporting documentation, including, but notlimited to ETO/HMIS.
  • Provide families withappropriate information (Section 8, lotteryapplications, CHAMP, income based rental listings, RAFT, SPI) tofurther assist in future housing needs.
  • Educate families onhow and where toaccess appropriate community resources, (i.e.,fuel assistance, food banks, thrift stores, donation centers, etc.)
  • Maintain and increase positive,team-oriented working relationships withlocal landlords, supportive service agencies, and state and non-profit agencies.
  • Responsible for tracking and reporting onoutcomes of preventionservices, including researchers.
  • Complete and maintain all necessary documentation in client’s records. This includes but is notlimited to ETO/HMISand researcher database; any additional reports requested.
Requirements:

This positionrequires 2-4 years ofexperience withhousing and/or social workand/or a bachelor’s degree in social work orrelated field. CPR certification is required and provided by FBMS. AEDand FirstAid mayalso be required and willbe provided by FBMS. Strongorganizational and communication skills arerequired. Applicant must have a valid driver’s license and daily access toa motor vehicle.
