Qualifications

• Case Manager certification within 120 days of start date
• CPR certification within 120 days of start date
• Driving positions require a minimum age of 21 and a current driver's license

Responsibilities

• Provides psychosocial rehabilitative services as described by Valley's Medicaid-based standards
• Supports clients in following individualized care plans and identifying case management goals
• Completes documentation on services provided within Valley's standards
• Meets productivity expectations as assigned by supervisor
• Distributes medications as necessary
• Transports clients to appointments as necessary
