Job summary

Valley Behavioral Health’s Adult Outpatient Valley West position is for a Case Manager who helps clients develop skills and resources for recovery, independence, and success. You will work with clients to promote progress within a supportive, goal-oriented framework.

Compensation varies by certification status, with uncertified roles starting at $17.50/hour and certified roles starting at $18.75/hour. Schedule: Tue–Fri, 9:30am–8:00pm; benefits include health, retirement, and professional

Qualifications

• Case management experience and related psychosocial rehabilitation skills.
• Strong documentation and communication abilities.
• Ability to transport clients and coordinate services as needed.

Responsibilities

• Provide psychosocial rehabilitative services per Medicaid guidelines.
• Support clients in following individualized care plans and document progress.
• Meet productivity expectations as assigned by supervisor.
• Distribute medications as necessary.
• Transport clients to appointments as needed.
